We are seeking a caring well-organized Case Manager to facilitate successful move-ins, promote housing stability, encourage recovery and independent living skills, and assist with appropriate transitions as needed.

The Case Manager is responsible for:

  • Conduct a thorough admission for new residents - review resident rights, responsibilities, and community living expectations
  • Complete initial and ongoing assessments of residents' needs, including housing stability, mental health, substance use recovery goals - meet regularly with residents
  • Coordinate continued funding for clients in residential program, obtaining written authorizations (including prior authorizations and continuing authorizations).
  • Ensure adequate documentation of committee activities for accrediting bodies and regulatory agencies
  • Build and maintain relationships with community partners, including shelters, treatment providers, healthcare providers, workforce programs, and social service agencies

Qualifications for the Case Manager

  • Bachelor's Degree in social services or a closely related field
  • A minimum of one year of experience as a case manager (or closely related position) in a social service environment
  • Prior experience working in residential programs
  • Demonstrated ability to successfully collaborate and work productively as a member of a multidisciplinary team.
  • Excellent written and oral communication skills and interpersonal skills
  • Ability to work effectively in a busy environment with multiple distractions. Ability to maintain focus and attention for prolonged periods.
