Join our team! We are seeking a caring and dedicated Full-Time Case Manager who is passionate about supporting individuals in achieving their goals and improving their quality of life.
The Case Manager is responsible for:
- Conduct thorough admissions and orientation, including facility tours, rights and responsibilities, treatment schedules, and expectations.
- Coordinate interdisciplinary assessments and assist in the development, implementation, and review of individualized treatment plans.
- Serve as the leading facilitator for patients, families, clinical staff, and funding sources during initial, monthly, and discharge conferences.
- Facilitate ongoing funding authorization, including documentation and communication with insurers and other funding sources.
- Lead clinical treatment planning sessions with clinical team.
- Maintain accurate and confidential clinical documentation, charting client progress and participation.
- Serve as a point of contact for families and caregivers, providing consistent updates, education, and discharge planning support.
Qualifications for the Case Manager:
- LSW credential or a Master's Degree in social work or closely related field.
- A minimum of three years of experience as a case manager (or closely related position) in a private or public rehabilitation center, healthcare setting, or social service environment.
- Prior experience working in rehabilitation program, day treatment, and/or residential programs for individuals with traumatic brain injuries.
- Demonstrated ability to successfully collaborate and work productively as a member of a multidisciplinary team.
- Excellent written and oral communication skills and interpersonal skills, including the ability to work well with a diverse range of individuals.
- Knowledge of disabled advocacy groups, accessible housing, and long-term alternatives for adults with traumatic brain injuries. Proficiency with MS Office Suite programs and standard business applications all pluses
