Overview

The Clinical Care Coordinator (C3) contributes to the development and implementation of program strategies, develops and maintains systems to effectively and efficiently utilize hospital resources and works for continuous improvement in customer service and processes.

EDUCATION REQUIRED - Graduate of an accredited school of Professional Nursing

EDUCATION PREFERRED - Bachelor of Science in Nursing

LICENSURES OR CERTIFICATIONS REQUIRED - Valid, active unencumbered Nursing license approved by the Georgia Licensing Board for GA locations and Alabama Licensing Board for AL locations BLS certification

LICENSURES OR CERTIFICATIONS PREFERRED - None

EXPERIENCE REQUIRED - Four years in communications, patient transfer, admissions or other related field OR an equivalent combination of education and experience

EXPERIENCE PREFERRED - Experience in a critical care or step-down setting or Case Management experience in similar areas

ROLE SPECIFIC ESSENTIAL DUTIES AND RESPONSIBILITIES -

1. Assigns working MS-DRGs and adjusts DRG as necessary based on patient conditions. Communicates any questions to unit assigned Utilization Review Nurse.

2. Facilitates patient throughput and coordinates bed assignment needs with Unit C3 or designee according to established milestones to reduce the number of patients holding in ECC, PACU, etc.

3. Oversees Care Coordination HUB activities.

4. Performs initial clinical review for direct admissions to determine if patient meets criteria for admission.

5. Initiates service recovery as needed.

6. Facilitates hospital transfer from other hospital inpatient stays. Verifies if care is considered higher level of care. Arranges communication for potential transfer to Floyd physician.

7. Receives calls for interfaculty transfers.

8. Assigns appropriate bed for admission and creates pending bed request in patient logistics system.

9. Sets isolation alerts and/or readmission alerts in patient logistic system for patients readmitted within the last 30 days, with a history of an isolation organism and/or admitted from a long-term care facility.

10. Facilitates report between sending and receiving nurse as needed.

11. Provides coordination and communication within the Unit C3 or designee regarding admission and bed assignments to ensure continuity through the continuum of care.

12. Reviews staffing for Care Coordination HUB to determine needs and anticipates staffing needs for future shifts.

13. Demonstrates technical and clinical competence and serves as a clinical resource person and role model for staff.

14. Participates on healthcare teams and committees as needed.

15. Performs C3 duties as appropriate.

16. Assigns breaks and meal periods for HUB staff.

17. Other related duties as assigned.

18. Communicates with physicians, staff, community and customers about the Care Coordination Hub.

19. Maintains and monitors required records and reports.

20. Facilitates patient throughput by working effectively with all members of the healthcare team.

21. Continuing Education Obtains unit specific required education Completes unit and departmental annual skills checklists.
