Summary of Position:
The Nurse Practitioner – Advanced Care and Hospice position provides direct patient care within state and scope of practice guidelines at assigned Barton Healthcare System outpatient clinic facilities in support of the Patient Centered Medical Home (PCMH) or Patient Centered Specialty Practice (PCSP). Assesses, plans, implements and evaluates care in collaboration with the multidisciplinary care team. Demonstrates the knowledge and skill necessary to identify and meet the individualized needs of the population served. This position relies on the appropriate authorization protocols when performing medical functions including diagnosing, treating and performing any other function within the scope of medical practice.
Qualifications
Education:
• Completion of an accredited Nurse Practitioner training program which has been accredited by the Committee on Accreditation of Allied Health Education Programs (CAAHEP);
Experience:
● Must possess recent clinical experience as a Nurse Practitioner with the exception of practitioners who have completed an approved training program within the past year.
● Minimum three years Outpatient Clinic, Advanced Care and/or Hospice experience preferred.
Knowledge/Skills/Abilities:
● Excellent verbal and written communication skills, with ability to adjust communication style for the target audience.
● Emotional stability conducive to dealing with a high-stress level as associated with dealing with fast paced physical activity, rapidly changing conditions, and demands of patients and staff required.
● Strong computer skills required.
● Demonstrates the ability to assess a situation from a variety of perspectives, considers various alternatives, and chooses the appropriate course of action.
● In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred.
Certifications/Licensure:
● Current National Certification as a Nurse Practitioner through the National Certification Corporation (NCC), American Nurses Credentialing Center (ANCC), or American Academy of Nurse Practitioners (AANPCB). Current California and Nevada licenses or ability to obtain prior to start date.
● Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider)
● Advanced Certified Hospice and Palliative Nurse preferred or will obtain within 2 years of employment.
● Valid California or Nevada Driver's license (depending on state of employee's residence) or ability to obtain prior to start date.
● Current vehicle insurance.

Essential Functions
1. Provides consistently exceptional care at all times.
2. Participates in initial assessment (from focused to comprehensive, depending upon the setting) and longitudinal care to the specified patient population. Provides a wide range of health care services to adult patients and seniors.
3. Takes detailed patient medical history, performs physical examinations and makes assessments and diagnosis. Coordinates a palliative treatment plan in coordination with the Medical Director, patient and interdisciplinary team.
4. Interprets health information and establishes diagnoses consistent with the accumulated data.
5. Formulates and implements the treatment plan, ensuring timely provision of necessary health care services.
6. Analyzes accumulated health information and collects additional observational, interview, historical, physical examination, and diagnostic data, modifying treatment as appropriate.
7. Reviews treatment and therapy plans and presents pertinent data in a manner meaningful to the Supervising Physician.
8. Orients, instructs and trains assigned personnel.
9. Operates equipment and performs job related duties in a safe manner. Ensures proper functioning of equipment and follows procedure when equipment malfunctions.
10. Provides direct patient care, evaluates outcomes, consults with physicians as required and adjusts care processes as indicated to ensure optimal patient care.
11. Ensures that all aspects of patient care and observations are documented. Documentation is complete, accurate and timely. Signs off medical record within 48 hours of seeing patients.
12. After reviewing with the Supervising Physician, initiates arrangements for admissions, completes forms and charts pertinent to the patient's medical record, and provides services to patients requiring continuing care, including patients at home.
13. Consults with the patient's attending physician, the hospice medical director and other healthcare practitioners regarding the patient's eligibility for hospice care, ongoing care needs and medical management related to the patient's terminal condition.
14. Completes the hospice face-to-face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification.
15. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
16. Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners.
17. Assures that patients receive appropriate measures to control symptoms, through collaboration with interdisciplinary team members.
18. Educates/counsels patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients. Serves as a nursing resource for consultation and education to members of the interdisciplinary team and other healthcare practitioners in the community.
19. Supports and upholds established organizational policies, procedures, and objectives related to quality improvement, safety, environmental and infection control.
20. Responds to the needs of the department by performing other duties, as necessary.
