Summary of Position: 

Speech Language Pathologist provides Speech and Language assessment and treatment to patients utilizing established policies and procedures.  Speech Language Services are provided based on the ages of the population served; Infant through geriatric - 65 years or older and in collaboration with the multidisciplinary care team.  The provider will assess, plan, implement and evaluate treatment programs to patients with speech, language, voice, swallowing and/or cognitive deficits. Speech Language Pathologists work in the following settings- Outpatient, Acute Care, Skilled Nursing, and Home Health to meet the needs of our patients. The Speech Language Pathologist will demonstrate the knowledge necessary to identify and meet the individualized needs of all patients served.  The Speech Language Pathologist is under the supervision of the Rehabilitation Services Manager and the Director of Rehabilitation Services.

Qualifications

Education:

● Completion of an accredited Speech Language/Pathology program.

● A graduate degree in Speech-Language Pathology

● A post-graduate fellowship

Experience:

● Previous experience as a Speech-Language Pathologist preferred.  New graduates will be considered if a Clinical Certificate of Competence supervision is needed.

Knowledge/Skills/Abilities:

● Highly effective written and verbal communications skills. 

● Proficient computer skills are required to document, communicate and enter information into the electronic medical records system.

● Requires critical thinking skills, decisive judgment and ability to work with minimal supervision.

● Must be able to work in a stressful environment and take appropriate action.

● In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred.

● Ability to provide speech therapy services to outpatient, inpatient-acute care, skilled nursing and home health populations.

Certifications/Licensure:

● Certification of Clinical Competence by the American Speech/Language Hearing Association.

● Current California and Nevada SLP licensure or ability to obtain prior to start date.

● Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date.

Essential Functions

1. Provides consistently exceptional care at all times.

2. Provides patient care, evaluates outcomes, consults with others as required and adjusts treatment plan as necessary.  Performs speech-language assessment/reassessment on patients/residents at designated and appropriate intervals, recognizing abnormal findings and parameters.  Based on assessed needs, develops an evaluation report to include an individualized treatment plan for each assigned patient within 24 hours.

b. Plan of Care, including short term and long term goals

c. Patient and caregiver training

d. Demonstrates skill, accuracy and ensures patient safety during therapeutic evaluations and intervention.  Perform bedside swallowing evaluations and interpret results.  Perform Video Fluoroscopic Swallow Study and interpret results.

e. Able to intercede with dietary plan of care, when revisions are necessary to ensure safety in patients who are at risk for aspiration.

3. Evaluates the patient's response to therapy and progress towards goals. Revises treatment plan based on patient response to treatment/ therapy.

4. Performs evaluations and treatment in accordance with pre-established protocols.

5. Acts as a professional resource to other members of the patient care team. Conduct internal training programs for the care team in speech-language activities and swallowing.

6. Keeps the patient/family informed of progress, procedures. Focuses on customer service needs. 

7. In collaboration with the multidisciplinary team, effectively educates patients, families and caregivers in treatments techniques and goals.  Participates in patient care conferences. 

8. Monitors and ensures accuracy of documentation in the patient medical record. All aspects of care delivered are accurately documented.

9. Operates equipment and performs job related duties in a safe manner. Insures proper functioning of equipment and follows procedure when equipment malfunctions. 

10. Orients, instructs and trains assigned personnel.

11. Demonstrates a working knowledge of regulatory and licensing standards and ensures that practice is compliant with standards. 

12. Responds to the needs of the department by performing other duties, as necessary.
