Private Duty LPN - Part-Time Weekend Shifts
Location:Union City, TN
Schedule:Every Saturday & Every Other Sunday, 7 AM - 6 PM
Patient Acuity:PEG Tube, Trach, Colostomy Care
Overview:We are seeking a skilledPrivate Duty LPNfor a part-time role providing high-quality, personalized care in Union City. This weekend position offers a balanced schedule and the opportunity to make a lasting impact on your patient's health and well-being. Key Responsibilities:
+ Perform PEG tube feedings and medication administration.
+ Manage tracheostomy care, including suctioning and monitoring.
+ Provide colostomy care and ensure the patient's comfort.
+ Assess and document changes in the patient's condition.
+ Communicate effectively with family members and the care team to provide comprehensive care.
Requirements:
+ Active LPN license in Tennessee.
+ Experience with PEG tube, trach, and colostomy care preferred.
+ Strong clinical skills and attention to detail.
+ Ability to work independently and maintain a compassionate, patient-first approach.

Responsibilities
+ Perform selected nursing procedures within the scope of LPN practice
+ Provide patient and family teaching according to plan outlined by the RN
+ Monitor patient's vital signs and make specific measurements and observations
+ Monitor and record intake and output. Report significant changes immediately
+ Observe, assess and record patient's physical and mental condition and reports any changes
+ Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
+ Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned
Qualifications
+ Licensed Practical Nurse / LPN license in good standing
+ Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
+ Active CPR certification
+ Familiarity with electronic health records
+ Strong problem solving and critical thinking skills
+ Reliable transportation, valid driver's license and current automobile insurance
