What You'll Do:
- Provide direct patient care: Assist with physical assessments, administer medications, and perform wound care, venipuncture, and urinary catheterization.
- Collaborate: Work closely with a healthcare team, including Registered Nurses, to implement and support personalized care plans.
- Patient & Family Education: Teach patients and their families about disease management, medication, and how to maintain safety at home.
- Document & Report: Maintain accurate records of patient care using electronic medical record systems and report any changes in patient condition.
- Ensure Safety & Compliance: Follow infection control protocols and meet regulatory requirements.
What We're Looking For:
- Licensure & Certifications: Active LPN license in Minnesota and Basic Life Support (BLS) certification. Valid Minnesota driver's license.
- Experience: Previous experience in home care or hospice preferred, but not required.
- Skills: Strong clinical judgment, time management, and excellent communication skills. Must be able to work independently.
- Physical Ability: Comfortable with tasks that require physical stamina, including standing, kneeling, and assisting with patient mobility.
