Sinai team. Hours for this position are Monday-Friday <8:30am – 5:00pm>. This position will include a hybrid of on-site, community-based, and remote work, including home visits and accompaniments to appointments.* The CVS - MSHP CM social workers are state licensed, and masters' level trained and members of the interdisciplinary care team. Using a strengths-based and person-in-environment approach, they utilize motivational interviewing skills to complete comprehensive, clinical assessments of patients that delve into unmet basic needs, social and emotional needs, connectivity with and experience of care, as well as strategies for managing illness/enhancing wellness. Intervention strategies include appointment accompaniment, community and home visits, collaboration with collateral contacts, and telephonic follow-up. The aim is to address any basic needs (e.g., food insecurity, housing, safety concerns), connectivity to resources (e.g., community supports, public benefits), experience of care (i.e., ensuring meaningful, culturally aligned connection with medical and mental health providers), and experience of illness (i.e. optimization of wellness given complexities of disease). Utilizes advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare through assessment and care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services to promote effective utilization of available resources and optimal, cost-effective outcomes. *REQUIREMENTS:* * LMSW or LCSW in the state of New York * Exceptional skills in engaging patients/families and utilizing critical thinking skills in assessment and coordination of resources within families and hospital and community networks. * Ability to develop, maintain and monitor connectivity to medical/behavioral health care, safety and wellbeing of patients. * Exceptional organizational, collaborative, and psychosocial assessment and intervention skills. * Ability to foster a supportive environment for patients and colleagues, and work collaboratively as part of a team. * Interest in and passion for being part of an innovative community-based program to improve the quality of care by addressing barriers to behavioral health, primary and specialist care, and avoid unnecessary Emergency Department visits and hospital admissions/readmissions. *Preferred Qualifications:* * Healthcare and/or community-based experience working with patients with complex chronic illness and/or behavioral health population. * Proficiency in MS Office. Ability to learn hospital documentation and scheduling systems and care coordination applications. * Bilingual (English/Spanish) preferred. * Case management and discharge planning experience preferred. *Vaccination:* As per New York State regulations, the successful applicant for this position must be vaccinated against COVID19 before commencing in the role. History and Physical, TB screening and vaccination records for Tetanus, Diphtheria and Pertussis (Tdap) vaccine and influenza for the current season must be provided or obtained upon hire. *Education* Master's degree in social work required. *Pay Range* The typical pay range for this role is: $60,522.80 - $129,600.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.    In addition to your compensation, enjoy the rewards of an organization that puts our heart into caring for our colleagues and our communities.  The Company offers a full range of medical, dental, and vision benefits.  Eligible employees may enroll in the Company's 401(k) retirement savings plan, and an Employee Stock Purchase Plan is also available for eligible employees.  The Company provides a fully-paid term life insurance plan to eligible employees, and short-term and long term disability benefits.
