Posting Summary Rutgers The State University of New Jersey is seeking a Health Care Case Manager for Newark Center City in the Department of Child Health Program at Rutgers School of Nursing, located in Newark, Heath Care Case Management position (HCCM) functions to support the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out-of-home placement, in collaboration with other staff of the Child Health Unit in that local office. Provides nursing case management services in home, local office and health care settings. Nursing case management services include nursing assessment of a child's health status and care needs, development of a health plan, maintenance of a health passport/health summary, ongoing monitoring, coordination of care, advocacy, and health education to assure that each child in out-of-home placement receives appropriate health care per
