We are seeking an organized and detail-oriented Intake Coordinator to join our team. The Intake Coordinator will be responsible for coordinating the intake process for new patients, ensuring accurate and timely documentation, and providing support to the clinical team. The ideal candidate will have experience in discharge planning, medical documentation, and working in a healthcare setting.

Review Cases for Wound Updates
Enter All Update and Notes in the KanTime Database
Enter Authorizations and Schedules in System
Obtain Re-authorizations for Additional Homecare Services
Follow-up on Requests From the PCP for Services
Contact and Receive Authorizations from Insurance Company
Contact Clinicians (PT's, OT's, ST's) to Schedule Services
Request Home Health Orders From PCP's, Hospitals, and Specialist
Review Charts for Procedural Accuracy and Efficiency
Receive and Forward Faxes to Correct Departments
Confirm Patient's PCP's
Assist Patient's with Any Questions, Comments, or Concerns
Develop Good Working Relationships with PCP Staff and Insurance Companies
Experience:
Previous experience working in a medical office or healthcare setting as an LPN
Working Knowledge of Microsoft Office (Word, Excel, Outlook, and PowerPoint)
Outstanding Customer Service Skills
Doctor Office Administrative Experience (Minimum 2-3 year)
