Van Dyk Health Care at Park Place- Assisted Living in Hawthorne, NJ is accepting applications for LPN- Licensed Practical Nurse * Per Diem *
Pay Rate = $38.00 per hour
Shifts: 6:00am-2:00pm / 2:00pm -10:00pm / 10:00pm - 6:00am
Shift Minimum: 2x a month
Duties & Responsibilities
Implement and maintain established nursing objectives and standards.
Make independent decisions concerning nursing care weekends *
Notify resident's physician and next of kin/sponsor, when there is a change in the resident's condition.
Maintain confidentiality of all resident care information. Supervise the self-administration of medications as ordered by the physician.
Receive telephone orders from physicians and record on telephone order form to be signed and returned within 48 hours.
Transcribe physicians to pharmacy order sheets, lab requests forms and MAR (medication Administration Record)
Perform routine charting duties in accordance with established procedures.
Sign and date all entries in resident's medical file.
Chart all accident/incidents involving residents and follow established procedures.
Perform day-to-day functions of the nursing activities in accordance with current rules, regulations and guidelines that govern this facility.
Perform administrative duties such as completing medical forms, reports, studies, charting, etc., as necessary.
Participate in surveys (inspections) made by authorized government agencies.
