Requirements:
1. Graduate of an accredited school of nursing with a current New York State License.
2. Valid NYS L. P. N
3. CPR Certification.
4. 1year experience in supervision preferred
5. 1year experience in a Long Term Care Facility preferred
Essential Functions:
1. Receive morning report and make out daily assignments.
2. Count with outgoing nurse, sign and assume responsibility for controlled drugs.
3. Make AM rounds to assess status of residents/patients on the unit, checking on specifics given during report. Alert CNA's of new treatments or instructions regarding resident care.
4. Make rounds with, or call physicians as necessary and chart accordingly.
5. Complete treatments for the unit according to standards of care.
6. Supervise the dining room during meals and assist as necessary.
7. Make PM rounds to check residents and rooms to prepare for report.
8. Assist Nurse Manager to formulate resident care plans.
9. Assure residents are taken to activities and staff participates when needed.
10. Perform emergency treatment as indicated (CPR, breathing tx, etc.).
11. Attend all mandatory inservices and other meetings and conferences as needed.
12. Schedule breaks for staff and self appropriately to ensure care of residents.
13. Identify and communicate ideas for operational changes, policy revisions and job responsibilities that will lead to cost savings, time savings, and/or improvement in resident/patient care.
14. Maintain a good working rapport with personnel within the facility to ensure that nursing activities and services can be adequately maintained to meet the holistic needs of the residents.
15. Assume a leadership role, accepting responsibility and accountability for the care of residents and function of the shift.

Physical Activity:
- Climbing: Must be able to ascend or descend stairs, ladders and the like, using feet and legs and/or hands and arms. Body agility is emphasized.
- Balancing: Must be able to maintain body equilibrium at all times to prevent falling.
- Stooping: Must be able to bend body downward and forward by bending spine at the waist, while performing essential job duties.
- May also have to bear weight 50 lbs. while stooping.
- Requires full motion of the lower extremities and back muscles.
- Kneeling: Must be able to bend legs at knee to come to a rest on knee or knees.
- Crouching: Must be able to bend the body downward and forward by bending leg and spine.
- Crawling: May be required to move about on hands and knees or hands and feet to retrieve objects under beds or desks.
- Reaching: Must be able to extend arm(s) in any direction above, below or at shoulder level.
- Frequent sitting at computer.
- Standing: Must be able to stand for sustained periods of time.
- Walking: Must be able to move about on foot [or with an assistive device(s)] to accomplish tasks.
- Must be able to walk throughout the facility.
- Pushing: Must be able to use upper extremities to press against something with steady force in order to thrust forward, downward or outward 50 lbs.
- Pulling: Must be able to use upper extremities to exert force in order to draw, haul or tug objects in a sustained motion 50 lbs.
- Lifting: Must be able to raise objects from a lower to a higher position or move objects horizontally from position-to-position that weigh 20 lbs.
