FlexStaff is seeking a Nurse Case Manager/Staff Nurse to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities. You will be responsible for traveling to Wappinger Falls and Garrison weekly.
The Nurse Case Manager will coordinate the delivery of health care services for persons receiving services residing in residential facilities.
Position Summary:
Coordinate and oversee healthcare services for individuals in assigned residences or program units. Perform nursing assessments and diagnoses, collaborating with the Interdisciplinary Team (IDT) to ensure positive health outcomes in compliance with OPWDD and relevant regulations.
Duties and Responsibilities:
1. Coordinate routine and specialty healthcare services, maintaining health profiles.
2. Assess and establish health care outcomes, updating semiannually or as needed.
3. Ensure medical, dental, and specialty appointments are tracked and scheduled.
4. Act as a liaison for health information among team members, families, and service providers.
5. Develop and update individual plans of nursing service (PONS), providing staff training.
6. Maintain accurate health care documentation and electronic health records.
7. Evaluate healthcare effectiveness, recommending alternative care if necessary.
8. Conduct health-related training for individuals, staff, and team members.
9. Assign and train Direct Care staff for healthcare procedures, monitoring performance.
10. Supervise AMAP staff and ensure certification and recertification.
11. Provide nursing coverage and complete handovers for on-call services.
12. Ensure compliance with agency policies and OPWDD regulations.
13. Participate in meetings and committees as assigned.
14. Coordinate with pharmacies for medication orders and administration.
15. Maintain and monitor medication areas and controlled substances.
16. Perform additional duties as assigned, including incident reports and policy development.
17. Comply with required training and corporate compliance programs.
Requirements:
1. Current NYS Registered Nurse license, Bachelor's Degree preferred.
2. Prior nursing experience or experience with developmental disabilities preferred.
3. Excellent oral and written communication skills.
4. Proficient in computer skills, including Microsoft Office and electronic medical records.
5. Ability to travel to multiple agency locations.
6. Flexible work schedule as approved by the Director of Nursing.
7. Full compliance with all laws, regulations, policies, and corporate compliance programs.

The salary range for this position is $88,000-$92,000/year
