Job Qualifications/Experience:
Graduate of an accredited nursing program. BS in nursing or related field. Masters preferred.
Current NJ RN license.
Experience in QAPI activities a plus
Physical Requirements as Defined by Occupational Safety
Duties & Responsibilities:
Promotes QAPI Agenda and Initiatives
Monitors OBQI
Monitors and coordinates leadership for process updates and Potentially Avoidable Events audits, monitors SHP follow up
Monitors and reports on Home Health and Hospice Compare
Participates in quarterly OASIS monitoring
Oversees occurrence reporting
Monitors websites for OASIS and payment structure updates
Participates in policy review and development
Assists in designing new processes
Responsible for advancing OASIS transfers and discharges weekly
Oversees Quality Committees
Runs Home Health QAPI Committee
Leads Fall Prevention Program
Spearheads PI Projects
Participates in Hospice quality initiatives
Oversees agency audits to identify trends and to promote integrity in clinical outcomes and billing practices
Organizes and participates in billing audits quarterly
Oversees and participates in monthly UR chart audits
Reports on audit findings at quarterly QAPI meeting
Assists field staff, managers, assistant managers, intake staff and CRC's in resolution of clinical, OASIS, Billing and Coding issues
Assists with discharge outcome reviews as per SHP reports weekly
Oversees and monitors all agency appeal processes
Manage Medicare and Medicaid audits and appeals
Assists with NOMNC Appeals
Manages the weekly OASIS transmission error follow up
Participates in all Joint Commission and DOH surveys
Assist with PPR completion
Assist with applications
PIP initiation and oversight
Assures that staff meets required competencies
Participates in orientation of new staff
Participates in the development of CRC's and Assistant Managers
Evaluates education needs of staff (ie - OASIS and regulatory) and works with education staff to present them to staff
Works to ensure compliance with regulatory and licensure requirements
Assists with growing agency business
Cross trains to learn new skills required for role
Accepts other assignments as requested to support agency
