**Prolastin Nurse Navigator- Remote**

**Shift:** **10:00 a. m.-7:00 p. m. Central Standard time.**

**Position Description**

**The Registered Nurse will provide clinical and operational processes related to coordination of care and utilization management for Specialty Pharmacy. The RN will be the primary point of contact for the program and work directly with patients and other stakeholders.**

**Licensure** **Current and Unencumbered Compact Registered Nurse licensure in your state of residence is required**

**Qualifications**

**Minimum of 5 years clinical experience in infusion, acute care, home care, specialty pharmacy, emergency room or critical care**

**Experience and knowledge of Medicare, Medicaid, prior authorizations or utilization management is preferred**

**Intermediate to advanced computer experience with multiple programs, including Windows-based applications and the ability to proficiently type.**

**Must be able to work independently and multitask in a busy environment.**

**Requires good communication, organizational and time-management skills**

**Must be comfortable taking inbound and outbound calls**

**Telephonic experience is a plus**

**Requires access to reliable and high-speed broadband internet in your home**

**Responsibilities:**

**Provide patient concierge services for new program referrals**

**Communicate with patients to provide specific program information**

**Provide patient education.**

**Act as liaison between patient and other stakeholders**

**Problem solving and handling of escalation of issues as needed**

**Document and report any patient or provider issues.**

**Collaborate with Pharmacist and other team members**

**Time Management: Ability to effectively manage one's time and resources to ensure that work is completed efficiently.**

**Excellent organizational skills.**

**Communication: Ability to provide clear verbal and written information to patients, co-workers, and leadership.**

**Critical Thinking and Decision Making: Ability to choose a course of action or develop appropriate solutions by identifying and understanding issues, problems, and opportunities.**

**Autonomy: Ability to work independently**

**Computer Skills: Proficient in computer literacy skills with ability to use and troubleshoot Microsoft Word, Excel, Outlook and PowerPoint**

If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.

For this position, we anticipate offering an hourly rate of 37 - 61 USD / hourly, depending on relevant factors, including experience and geographic location.

This role is also anticipated to be eligible to participate in an annual bonus plan.
