We offer an annual salary of $80,000 – $90,000 and a year 1 bonus potential of up to $15k

Spring Mill Senior Living, located in Phoenixville, PA, is hiring a Director of Health & Wellness to lead the clinical team in our Assisted Living and Memory Care community.

Your Role: Director of Health & Wellness

As Director of Health & Wellness, you will oversee the health, safety, and wellness of all residents in the community. This hands-on leadership role requires a clinical expert who can manage compliance, guide staff, communicate effectively with families, and ensure residents receive the highest level of compassionate care. You'll serve as the clinical leader and primary liaison between residents, families, physicians, and the broader team.

What You'll Do

Clinical Oversight & Regulatory Compliance

  • Ensure the community remains compliant with all federal, state, and local regulations related to clinical care
  • Lead the collection, analysis, and reporting of clinical incidents and trends, implementing improvement plans as needed
  • Collaborate with the Administrator and team leaders to uphold resident health and safety standards
  • Partner with pharmacy consultants to ensure optimal medication management and pharmaceutical care

Resident Care Management

  • Oversee resident assessments and the development of personalized service plans, including updates related to health status changes
  • Ensure timely and accurate completion of monthly resident updates and changes in condition reporting
  • Coordinate with other departments on changes in services or medications and ensure care plans reflect those updates
  • Maintain communication with residents' families/responsible parties regarding care changes and health concerns
  • Notify the Business Office of any fee adjustments due to changes in care needs

Leadership & Team Support

  • Supervise LPNs, CNAs, and other care team members to ensure delivery of consistent, high-quality care
  • Provide coaching, mentoring, and support to clinical team members, upholding company policies and regulatory standards
  • Create and manage monthly clinical staffing schedules
  • Participate in the clinical on-call rotation as needed
  • Ensure adequate medical supplies are available and maintained

Community Engagement & Communication

  • Serve as a clinical representative during community awareness and outreach efforts
  • Participate in leadership meetings and collaborate cross-functionally with department heads
  • Contribute to ongoing education by assisting with training or presentations as requested

What You Bring

  • Licensed Practical Nurse (LPN) in good standing
  • Minimum of 1 year in a nursing leadership role (senior living experience preferred)
  • Current BLS (Basic Life Support) certification
  • Strong communication, organizational, and team leadership skills
  • A proactive and resident-centered approach to care
