We are currently seeking a compassionate and skilled Full-Time Licensed Vocational Nurse (LVN) to join our dynamic team serving Plano and the surrounding areas.
**Job Summary:**As a Full-Time LVN in our Home Health environment, you will play a vital role in providing quality medical care to patients in their homes. Your responsibilities will include assessing patient health needs, implementing care plans, administering medications, and collaborating with other healthcare professionals to ensure the best possible outcomes for our patients.
**Key Responsibilities:**
- Provide direct patient care as directed by the care plan, including administering medications, wound care, and other clinical tasks.- Conduct patient assessments and monitor vital signs to evaluate patient health and report any changes to the supervising RN or physician.- Collaborate with the healthcare team to develop, implement, and update individualized care plans.- Educate patients and their families on health management and wellness, providing resources and support as needed.- Maintain accurate and complete patient records and documentation in compliance with regulatory requirements and company policies.- Ensure a safe and clean environment for patients by following infection control protocols and safety procedures.- Participate in continuous education and training programs to enhance professional skills and stay updated on best practices in home health care.- Communicate effectively with patients, families, and interdisciplinary team members to ensure coordinated care.
**Qualifications:**
- Current and valid LVN license in the state of Texas.- Previous experience in home health care or a similar setting is preferred.- Strong clinical assessment and critical thinking skills.- Excellent communication and interpersonal skills.- Ability to work independently and manage time effectively.- Compassionate, patient-centered approach to care.- Valid Texas driver's license and reliable transportation.
- Experience in Wellsky EMR a plus.
