Discovery At Home is hiring a Full Time Registered Physical Therapist for our West Side communities and outlaying areas.
In this role you will provide skilled Physical Therapy services on an intermittent basis to clients in their homes in accordance with physician orders.
Responsibilities:
- Provides initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
- Assists in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
- Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests.
- Treats clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, water, light and electricity.
- Consults and familiarizes staff regarding the established plan.
- Teaches, supervises, and counsels the family and client in the physical therapy program.
- Instructs client in care and use of wheelchairs, braces, crutches, canes and prosthetic and orthotic devices.
- Provides the equipment required to carry out the plan.
- Arranges for out-patient service for procedures which cannot be given at home.
- Records on a timely basis all evaluation data, treatments, and client's response to therapy intervention. Also records quality assurance data.
- Prepares an assessment of the client's condition and periodic evaluations and summaries.
- Supervises Physical Therapy Assistants and paraprofessionals at least once monthly.
- Records and reports to the physician the client's reaction to treatment and /or any changes in the client's condition.
- Communicates with the physician at least every 30 days or whenever changes occur regarding the client's status and obtains additional orders, if necessary.
- Communicates with other team members (including Home Health Aides) and, when appropriate, instructs them in certain phases of physical therapy that they may use while working with the client.
- Writes clinical/progress notes on the date of the visit and incorporates same in the client's record within 72 hours.
- Participates in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary.
- Supervises the home health aide every two weeks when physical therapy is the only professional discipline involved in the client's care and supervises the Physical Therapy Assistant at least monthly.
- Participates with staff, clients, and physicians in discharge planning activities and completes a Physical Therapy Discharge Summary.
- Understands and adheres to established policies and procedures of agency.
- Participates in Quality Assessment and Performance Improvement activities as assigned.
- Attends all patient care conferences as scheduled.
- Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes and significant change in condition in accordance with agency defined time frames.
- Obtains a medical history, particularly as it relates to the present condition.
- Evaluates the client, the client's significant other(s) and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the client, and what other Agency/community services will be required.
- Assures that the physician's orders are appropriate and discusses necessary changes.
Qualifications:
- Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
- Must be licensed or registered by the State.
- Must have at least one year of institutional experience.
- Must have at least 2 years of home care experience and at least one year supervisory experience. (2years preferred)
