Job Summary:

The Patient Navigator, Population Health works closely with primary care physicians and various specialty physicians and services to maximize the health status of patients. This position requires a high frequency of contact with the very high risk patients and their care givers for education of health promotion and self-care skills as well as facilitating compliance with the required quality measures for all population patients. The Patient Navigator will also ensure the coordination and communication of a patient's treatment plan and general status to all care givers during all transitions of care. This position requires advanced clinical knowledge and expertise to identify and implement improvement processes and the ability to direct staff across settings and implement care coordination plans both in acute care, post-acute, home and out-patient settings.

Role Responsibilities:

  • Conduct comprehensive patient assessments (in person or by phone) to identify needs, close care gaps, and prevent deterioration in high‑risk populations.

  • Coordinate multidisciplinary care plans, collaborating with physicians and care teams to ensure adherence to treatment goals, smooth care transitions, and appropriate referrals.

  • Educate patients and caregivers, assessing learning needs and delivering tailored teaching, materials, and decision‑support tools.

  • Monitor and manage care delivery quality, including home care and ancillary services, tracking outcomes, ensuring compliance with quality measures, and supporting continuous improvement.

  • Support operational responsibilities, including accurate data reporting, attending trainings, maintaining schedule flexibility, traveling to patient locations when needed, and participating in on‑call rotations.


 Qualifications:

  • BSN. Master's Preferred.
  • Valid NJ Registered Nursing License.
  • Must be fluent in Korean and English
  • Highly developed interpersonal skills including motivational interviewing skills, in order to interact effectively and motivate patients to change behavior when necessary.
  • Must be proficient at multi-tasking and prioritization working in a high volume environment with little supervision.
  • Ability to work independently as well as in a team environment.
  • Excellent analytical and deduction skills.
  • Good reasoning and problem solving ability and be able to take initiative in finding solutions to difficult and/or sensitive problems.
  • Must be organized and able to multi-task.
  • Ability to work in stressful situations and be flexible to accommodate patient and family needs.
  • Demonstrated proficiency and comfort with PC and Microsoft Office skills.
  • Current valid Driver's license and auto insurance.
  • Experience in a variety of patient settings is recommended.
  • Knowledge of community resources
  • Experience in home care, discharge planning preferred

 

Englewood Health job postings provide a minimum rate to comply with New Jersey Law. The rate of pay for each position will be determined based on a variety of factors including the candidate's relevant experience, qualifications, skills, etc. The minimum rate does not include bonuses/incentives, differential pay or other forms of compensation. In addition to the minimum rate, eligible employees may receive comprehensive benefits, including health insurance, retirement plans, paid time off, and other wellness resources.

Minimum rate: $45.19 - $52.88 per hour
