Job Summary:
The Population Health Coordinator, Community, is responsible for coordinating system-wide health initiatives within Englewood Health, The Shirvan Family Live Well Center (LWC) and the broader community. They are an integral member of the Population Health team and provide front-line support for the development and management of multiple programs. The Coordinator will be responsible for researching, coordinating, implementing, managing, and evaluating equity based wellness programs. They will be accountable for the day-to-day operations of these programs and must be proactive and possess the ability to multi-task, problem solve, with strong attention to detail. The Coordinator will be responsible for specific administrative tasks including knowledge for collecting and organizing data and providing data analysis and summary presentations. They will also support the Community Needs Assessment, as needed, as well as providing part time support to day-to-day operations of the LWC, also to include assistance in planning and coordinating program development, and coordination with local organizations.

Job Qualifications: 

  • A minimum of two (2) to four (4) years of relevant professional experience required
  • Demonstrated interest in and experience working within Community Health, Public Health, or a related health field
  • Excellent verbal and written communication skills, with the ability to effectively engage diverse audiences
  • Strong cultural awareness and a commitment to fostering inclusive and equitable environments
  • Proven track record of professionalism, accountability, and high-quality performance
  • Exceptional organizational skills with strong attention to detail
  • Ability to effectively manage multiple priorities and meet deadlines in a fast-paced environment
  • Experience working collaboratively on multidisciplinary teams and projects, demonstrating flexibility and teamwork

Education Requirements:

  • Bachelor's degree required

License/Certification/Registry:

  • Current driver's license required and access to a vehicle

Englewood Health job postings provide a minimum rate to comply with new Jersey Law. The rate of pay for each position will be determined based on a variety of factors including the candidate's relevant experience, qualifications, skills, etc. The minimum rate does not include bonuses/incentives, differential pay or other forms of compensation. In addition to the minimum rate, eligible employees may receive comprehensive benefits, including health insurance, retirement plans, paid time off, and other wellness resources.
 Minimum rate: $25/hour
