JOB DESCRIPTION

The Diabetes Nurse Educator provides diabetes expertise to medical and clinical staff, patients and families. They assess the clinical status of referred patients, consult with hospital and physician office personnel in coordinating a plan of care and providing self-management education. Relies on experience and judgment to accomplish responsibilities. Proactive with capability to work under minimal supervision. Creativity is expected to perform job. May lead and direct the work of others.

RESPONSIBILITIES
  1. Diabetes Education
    1. Assists in planning, coordinating implementing and evaluating the activities and curriculum of the Diabetes Education Program in a manner consistent with hospital standards and efficiently maintains appropriate documentation of educational/medical records and statistics in order to ensure national program recognition status.
    2. Serves as a member of the Diabetes & Nutrition Center by actively participating in planning, coordinating, implementing and evaluating the Diabetes Education Program and developing short- and long-range program goals and departmental policies and procedures that promote the hospitals mission and philosophy.
    3. Efficiently performs assessments of referred patients in a caring manner, educates and evaluates participants and serves as a resource to patients upon program completion.
  2. Quality
    1. Performs high quality assessments of infant, pediatric, adolescent, adult and geriatric populations and maintains awareness of the age of the patient served when documenting, planning care and providing follow up services.
    2. Observes and adheres to quality standards established by the department. Participates in continuous quality improvement programs in order to foster the provision of quality health care services by the department and hospital and ensures that all serves are provided with compassion, understanding and respect for others.
    3. Plans and coordinates high quality continuing education programs in order to ensure that hospital personnel receive high quality clinical training and education. Act as a clinical consultant to hospital staff in a manner that promotes the provision of high-quality care by the hospital staff.
    4. Promotes the quality and efficiency of his/her own performance by remaining current with the latest trends in field of expertise through participation in job-relevant seminars and workshops, attendance at professional conferences and affiliations with national and state professional organizations.
  3. Collaboration & Partnership
    1. Develops positive relationships between the hospital and community agencies, special interest and civic groups and professional affiliations by acting as hospital representative. Works through civic groups and public education activities to improve community health and foster the hospitals image as a leader in the health care industry.
  4. Other Duties as Assigned
    1. Performs other duties as assigned or requested.
QUALIFICATIONS
  • 5 years clinical experience as a Registered Nurse
  • Active Certification as Certified Diabetes Educator (CDE) required.
  • Bachelor's Degree in Nursing
  • Good oral & written Communication, interpersonal, critical thinking, organizational, time management & computer skills
  • Current unrestricted Mississippi RN license; BLS
