The Holyoke Visiting Nurses Association, Inc. is looking to hire a Hospice Nurse Coordinator Full-Time to work under the direction of the Hospice Manager and within established organizational and departmental policies and procedures, responsible for providing skilled care and case management to clients/families in accordance with the State Nurse Practice Act and accepted professional hospice standards of practice. #The Hospice Nurse Coordinator provides services using best practice guidelines, communicates/documents observations and assessments, promotes and maintains quality of life for clients/caregivers through direct care, teaching, and counseling following the interdisciplinary group plan of care. The Hospice Nurse Coordinator assists with the orientation and professional development of team members and provides clinical oversight to the on-call and weekend nurses, in conjunction with the Hospice Manager. Make an assigned initial/admission visit to a prospective client#s home to: Explain policy, services and limitations to clients/caregivers; Assess the needs of client/caregivers to establish the appropriateness of care; Obtain informed consent signatures of client/caregivers and physician; and Explain client#s rights. Develop a comprehensive nursing assessment through collection of client/caregiver data, within five days of admission that includes: Client#s physical, emotional, spiritual, social, psychological, bereavement, and cultural status; Coping strategies, support systems, and learning needs of client/caregivers; Safety in a hospice setting; and Reassessment in a systematic and ongoing manner. Analyze the assessment data to determine nursing diagnoses: Identify problems that may be resolved, diminished or prevented through nursing and/or interdisciplinary intervention; Validate diagnoses; and Form the initial outline for the interdisciplinary group (IDG) care plan in light of the diagnoses. Identify expected outcomes individualized by client/caregivers. #Implement IDG Plan of Care: Provide direct skilled nursing care and emotional support; Maintain accountability for all aspects of client#s care outcomes, including supervision of HHAs; Communicates to other IDG members so care reflects hospice#s interdisciplinary practice; Updates Plan of Care as needed, at a minimum every 15 days; Assures interventions are consistent with established plan of care and best practice guidelines; Ensures availability and accessibility of nursing care twenty-four (24) hours a day, seven (7) days a week; Interventions are documented in a timely manner. Evaluate client/caregiver#s progress towards attainment of outcomes. Document all nursing visits, telephone calls, and IDG collaboration in a timely manner and according to agency policies and procedures. Maintain established agency, hospital and departmental policies and procedures, objectives, confidentiality, quality improvement program, compliance, safety, infection prevention and environmental standards. Maintain professional growth and development through attendance at seminars, workshops, conferences or in-services, professional affiliations or journals to stay abreast of current trends in the field of expertise.# Meet annual competency and retraining requirements.# Maintain any certifications required for the Hospice Nurse Coordinator position. Attend weekly IDG meetings. Attend other staff and agency meetings as required. Provide orientation and ongoing education to team members on policies and procedures, clinical information, and computer documentation.# #Make supervised visits to new and existing staff members to provide clinical instruction and support. On a rotating basis, provides clinical oversight and direction to the on-call and weekend nurses, in conjunction with the Hospice Director. Perform other functions/duties as requested. MINIMUM REQUIREMENTS: Graduate of an approved, accredited school of professional nursing, BSN required.# Current licensure to practice as a Registered Nurse in the Commonwealth of Massachusetts is required.# Current CPR certification required. Minimum of one (1) year of medical/surgical nursing and hospice/home care nursing experience required; supervisory, educational, and/or quality department experience preferred. Demonstrated excellent verbal and written communication skills required.# Demonstrated strong interpersonal, organizational and time management skills required. Demonstrated knowledge and understanding of the dying process with an ability to identify and treat signs and symptoms of the same, including competency in pain and symptom management required. A current driver#s license, good driving record and reliable transportation are required.
