Job Summary:
Provides case management for transplant candidates and/or clinical management of post-transplant patients. Collaborates with providers and a variety of departments to coordinate the plan of care and provides expertise in transplant care and treatment. Accountable for ensuring the effective coordination of transplant related patient care during the pre-transplant, transplant and/or post-transplant periods.
Essential Responsibilities:


  • Reviews referrals for transplant against patient selection criteria.

  • Develops and implements case management programs for patients awaiting transplantation.

  • Coordinates all phases (pre-transplant, post-transplant) of care with different departments within KP Hawaii to facilitate transplant services.

  • In collaboration with the KFHP Travel team, facilitates transportation and housing arrangements as required (pre-transplant only).

  • Reviews and recommends authorizations for referrals for transplant related care at the contracted Centers of Excellence (COEs).

  • Acts as liaison between Kaiser Permanente and the transplant centers that are affiliated with Kaiser Permanente and requires to be available for consultation with coordination of transplant to or from Kaiser Permanente Hawaii region.

  • Coordinates an interdisciplinary approach to providing continuity of care for transplant candidates and posttransplant patients, including utilization management, facilitation of discharge planning and clinical management of transplant related follow up care.

  • Consults with other transplant team members as needed to ensure safe repatriation and appropriate monitoring and treatment plans following transplant.

  • Provides patient orientation and education regarding post-transplant care upon repatriation after transplantation.

  • Establishes and maintains effective partnerships between Kaiser Permanente and contracted transplant sites. with the overall goal of providing total quality management for Kaiser Permanentes transplant patients.

  • Conducts concurrent and retrospective review of post-transplant admission(s) and potential quality events.

  • Maintains up-to-date knowledge of changing technology related to transplantation services, and KP benefit and policy changes which impact administration of transplantation services.

  • Develops and updates educational materials for the KP transplant program including but not limited to educational materials for members, families, and physicians.

  • Participates in KP National Transplant Networks educational activities, collaborates as appropriate on projects and program development activities.

  • Ensures that accurate clinical records are maintained for transplant related care, including review of the COEs EMR and transcription of clinical documentation as required.

  • Adheres to policy and procedures specific to the transplant program operations.

  • Recommends new policies and supports the ongoing review and revision of existing policies and procedures.May perform patient care to the extent necessary to maintain clinical expertise, competency, and licensing necessary to fulfill job responsibilities and to direct the provision of care on the unit.

  • Provides direct patient care on an as needed basis. Provides services that are within scope of license and in compliance with all legal, regulatory, and policy requirements relevant to clinical role performed.

  • Incorporates the KP Nursing Vision, Model and Values throughout their Nursing Practice.


Basic Qualifications: Experience

  • Minimum five (5) years of RN experience in any of the following: critical care, intensive care unit (ICU), emergency care, general medical/surgical services, acute care, dialysis or transplant care services.

Education
  • Bachelors degree in nursing or health related field. Graduate from an accredited school of nursing. High school diploma or General Education Development (GED) required.
License, Certification, Registration
  • Registered Nurse License (Hawaii)
Additional Requirements:
  • Demonstrated ability to work effectively across medical specialties, communities, departments and regions/divisions.
  • Demonstrated ability to communicate effectively with physicians and other clinical providers, as well administrators.
  • Demonstrated skills in critical thinking, problem solving and clinical case management.
  • Must be able to work in a Labor/Management Partnership environment.
Preferred Qualifications:
  • Minimum one (1) year of experience in clinical transplant coordination or case management specializing in any of these areas: disease, population or chronic disease management.
  • Masters degree preferred.
