**Job Title: Licensed Practical Nurse (LPN) - Full-Time Evening Shift**

**Job Summary:**
Rittenhouse Village at Northside is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our nursing team for the evening shift. **Key Responsibilities:**
- Deliver direct patient care following established standards of practice, policies, and procedures.
- Assist registered nurses (RNs) and physicians in administering medications, treatments, and procedures.
- Monitor patient vital signs, report any changes in patient condition, and document care provided in accordance with facility protocols.
- Conduct patient assessments and contribute to the development and implementation of individualized care plans.
- Educate patients and their families regarding health conditions, medications, and post-discharge care.
- Maintain a safe, clean, and organized work environment, adhering to infection control protocols.
- Collaborate with multidisciplinary teams to ensure coordinated care and optimal patient outcomes.
- Participate in ongoing education and professional development to enhance nursing skills and knowledge.
**Qualifications:**
- Current and valid LPN license in Indiana.
- Completion of an accredited practical nursing program.
- CPR and First Aid certification.
- Previous experience in a healthcare setting is preferred but not mandatory.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment.
