MedStar Health is looking for a Case Management Assistant to join our team at Washington Hospital Center!
The Case manager assistant Implements the plans and interventions defined by the Case Managers and Clinical Social Workers. Under the direction of the case management staff, performs multiple activities that support the movement and transitions of patients out of the inpatient hospital setting. Assists in the administration of Case Management functions to include, but not limited to –organization of work flow, communications, links to community resources, transportation, and other duties to assist in the facilitation of discharges. Provides clerical and support functions to the case management staff members with the goal of promoting expeditious patient transfers and/or discharges. Maintains a high degree of confidentiality with sensitive patient information. Position is hybrid based on leadership discretion and operational needs.

Primary Duties:
Coordinated post-acute care placements, including acute rehabilitation, skilled nursing, hospice, home health, dialysis, psychiatric, long-term care, LTACH, and DME services, ensuring seamless transitions through discharge.
Obtained insurance authorizations and collaborated with hospitals, payers, and post-acute providers to facilitate timely approvals and continuity of care.
Managed patient discharge logistics by arranging medical transportation, preparing discharge packets, delivering required Medicare and observation notices, and maintaining accurate electronic documentation.
Partnered with multidisciplinary teams, including Case Managers, Social Workers, Utilization Review Nurses, Transitional Care Nurses, and community providers, to identify and resolve discharge barriers.
Maintained compliance with regulatory, accreditation, quality, and safety standards while supporting department initiatives, tracking discharge metrics, and promoting exceptional patient-centered service.
Qualifications:
High School Diploma or GED (Required)
Bachelor's degree (Preferred)
Medical Terminology Knowledge (Preferred)
This position has a hiring range of
USD $18.70 - USD $32.72 /Hr.
