General Summary of Position
Facilitates the inpatient admission of appropriate rehabilitation patients to MedStar National Rehabilitation Network (MNRN) by obtaining accurate clinical data through performing on-site and record/telephone patient evaluations. Individuals in this role are responsible for educating the referring staff of community hospitals, patients, family members and external payers regarding the effective and efficient utilization of MNRN program services and resources.


Primary Duties and Responsibilities

  • Performs on site, record and telephone pre-admission screening/clinical assessment to determine appropriateness of referral and medical stability for inpatient acute rehabilitation program.
  • Completes assessment/evaluation within 24 hours or in one working day of the referral (documents exceptions within the electronic medical record).
  • Reviews clinical information in patient's medical record and performs physical assessment of the patient to verify/confirm information in medical record and interviews referring healthcare team members, patient's and family.
  • Prepares a summary report at time of initial on-site evaluation and provides preliminary recommendation to referral source regarding appropriate level care of potential patients.
  • Reviews clinical data/assessment with MNRN manager and/or physician for determination of medical stability and appropriateness for admission and notifies referral source of MNRN decision.
  • Educates patient, families or significant others, referral sources, hospital personnel, external payers and case managers regarding inpatient acute rehabilitation program service and benefits, admission criteria, bed availability, day treatment programs, outpatient centers/programs, and basic knowledge of third party reimbursement.
  • Maintains marketing plans for each assigned hospital. Maintains a thorough knowledge of MNRN facilities, products, and services while developing and maintaining ongoing relationships with referral sources.
  • Prepares/submits marketing reports and all necessary data for accurate maintenance of statistics, conducts tours for family members and referral sources and participates in formal marketing programs as directed.
  • Participates in service area activities, Quality Improvement projects and MNRN program steering committees.
  • Provides feedback on admitted patients' progress at MNRN to referral sources as requested.
  • Serves as a rehabilitation nurse consultant/resource for referring hospital personnel and maintains confidentiality of all hospital and patient information and high ethical standards which entail providing recommendations for alternative level of care when patient needs cannot be met at MNRN.
  • Submits preliminary treatment plan, estimated length of stay and rehabilitation goals required for precertification process. Communicates verbally with appropriate MedStar NRN staff regarding


     

    Minimum Qualifications
    Education

    • Bachelor's degree as an RN required and
    • degree in allied health required and
    • Will consider relevant work experience in lieu of education. required

    Experience

    • 1-2 years of RN experience required and
    • Allied health experience where the individual has demonstrated knowledge ofhealth care delivery systems, rehabilitation medicine, and third party payers reimbursement issues. required

    Licenses and Certifications

    • Current RN license issued by the state required at the time of hire. required and
    • Individuals working in DC will have to obtain their D.C. license within the first 90 days of employment. required and
    • Must have access to reliable transportation.

    Knowledge, Skills, and Abilities

    • The incumbent in this role must have thorough understanding of the process of rehabilitation, expert physical assessment and patient/family education skills, the ability to be sensitive to confidential information, and a professional and compassionate attitude towards patients, family members, staff and external parties.
    • The incumbent in this role must be able to operate basic office equipment including but not limited to: phone, laptop, tablets, fax and copying machines.


    This position has a hiring range of $72,758 - $123,260
