MedStar Health is looking for a Therapy Clinical Mentor to join our team at MedStar Health Home Care! The ideal candidate will be a registered Physical Therapist with demonstrated knowledge of home equipment, home care reimbursement, and regulations governing Physical Therapy Assistants, Licensed Practical Nurses, and Home Health Aides.
As a Therapy Clinical Mentor, you will be responsible for coordinating patient care with designated clinical discipline, under the supervision of the Operations Director. Assists in the clinical distribution and proper care of patients by supporting field staff indecision making, case conferencing and daily management of schedules.

Primary Duties:
Acts as coverage to support operations in absence of Operations Director. Assists with patient complaint resolution. Communicates with patients, caregivers, and interdisciplinary staff to resolve actual or potential issues with the patient. Conducts chart reviews and assures that admissions, recerts, and discharges in the patient documentation are accurate and that proper utilization and clinical needs are being achieved.
Leads preceptor responsible for training and providing feedback to new associates, doing peer-to-peer reviews, and being available as a clinical resource for field staff. Maintains a caseload of patients and meets the expectation of care described in the corresponding clinical job description. As deemed necessary, the role of Care Manager also performs admissions (as appropriate), revisits, recerts, discharges, and joint visits to ensure quality patient care delivery and excellence in patient experience.
Maintains effective working relationships with other departments. Participates in multidisciplinary quality and service improvement teams, meetings, and on committees representing the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations. Manages care of patients to maximize patient care services and maximize patient experience. Participates in Mock surveys as directed.
Performs Daily Workflow duties as designated per team. Provides an initial review for 485 plans of care consistently across the agency and provides feedback as needed on post-review status at the discretion of the Operations Director. Provides Operations Director with input regarding team members' work performance. Provides oversight of staffing and scheduling concerns.
Provides weekly case conferences to orienteers during IPR and continues bi-weekly after the IPR period. Responsible for clinical outcomes and utilization management. Serves as Clinical resource to patient, caregiver, physician. Serves as an educator in the discipline, providing feedback and suggestions to improve workflow, best practices, and standard procedures.
Qualifications:
Bachelor's degree, master's degree (MPT), or Doctoral degree in Physical Therapy (DPT).
1-2 years' experience in a certified home care or home hospice program.
PT (Physical Therapist) License in all jurisdictions in which the employee provides services to patients, the District of Columbia, Maryland or Virginia or any combination based on work location(s).
CPR (Cardiac Pulmonary Resuscitation).
Valid State Driver's License in good standing and a dependable vehicle for transportation.
