Primary Duties:  

• Conducts psychosocial assessments and coordinates timely referrals to community resources, helping patients adapt to lifestyle changes and reintegrate into the community
• Provides crisis intervention, therapeutic support, and advocacy for patients and families, ensuring their social, emotional, and financial needs are addressed.
• Collaborates with healthcare teams during multidisciplinary rounds to develop and implement comprehensive discharge and care plans.
• Maintains accurate documentation in medical records and ensures compliance with departmental policies and regulatory standards.
• Contributes to quality improvement efforts, supervises staff or students, and participates in hospital committees and community outreach initiatives.

Qualifications: 

• Master's degree in social work from a school accredited by the Council on Social Work Education. (Required)
• 1-2 years- Experience in social work in a hospital setting (Required)

Licenses/ Certifications:

·       LGSW- Licensed Graduate Social Worker in the State of Maryland (Required)

This position has a hiring range of $60,632 - $107,494
