General Summary of Position
Collaborates with providers to ensure safe and effective use of specialty medications and improves communication among the patient's healthcare team by reviewing and evaluating clinical guidelines, formulating protocols, conducting medication reconciliation, creating patient care plans and providing patient consultation that will improve patient medication adherence, health, well-being and knowledge. Documents and reports on healthcare-related interventions and outcomes. Participates in the process of maintaining specialty pharmacy accreditation.
Primary Duties and Responsibilities
Minimum Qualifications
Education
- Doctoral degree from an accredited college of Pharmacy (PharmD) required
Experience
- 3-4 years of direct patient care experience in a clinical role, preferably in a specialty pharmacy required
- Ambulatory Care experience preferred
Licenses and Certifications
- PHARMD - Licensed Pharmacist in the District of Columbia as applicable to practice site required
- Eligibility for reciprocity to surrounding states as required for practice required
- Completion of an ASHP accredited residency or other approved advanced training program preferred
- Board of Pharmacy Specialty certification (BPS) or other approved certification (i.e., Certified Specialty Pharmacist (CSP), HIV Pharmacist (AAHIVP)) preferred
This position has a hiring range of $118,331 - $230,172
