Overview:

• The Diabetes Care Nurse will work in active partnership with the patient, family, provider, and clinical staff to develop and implement a well-coordinated and innovative plan of care that ensures appropriate education, goal setting, control and maintenance of their disease.
• This position is integral to Mercy's efforts to provide high-quality patient care and implement successful care delivery methodologies needed for alternative payment models and value-based care.

Qualifications:

• Education: RN with a BSN or equivalent experience required.
• Experience: Prior work experience in Case Management, Disease Management, or Managed Care.
• Certification: Case Management certification preferred.
• Licensure: Current RN licensure in the applicable state of practice required.
