Join Mercy's Growing Palliative Care TeamMercy Hospital Jefferson is seeking a compassionate and patient-centered Nurse Practitioner or Physician Assistant to join our established outpatient Palliative Care program. This is an excellent opportunity for an advanced practitioner who is passionate about improving quality of life, guiding patients through goals-of-care discussions, and providing holistic support for individuals with serious illnesses.

Position Highlights- Monday-Friday clinic schedule
- Clinic hours: 7:30 a.m. - 4:00 p.m.
- Part-time position - 32hrs/week
- Outpatient-focused practice
- Average of 8 patients per day
- Providers manage their own patient panel
- Minimal call responsibilities with infrequent urgent patient needs
- System-wide EMR

All patient care is delivered in the outpatient setting, with inpatient involvement limited to communication regarding outpatient referrals.
Responsibilities- Conduct comprehensive palliative care assessments
- Facilitate goals-of-care and advanced care planning discussions
- Manage symptoms and improve quality of life for patients with serious illnesses
- Collaborate with physicians, nursing staff, social workers, and other interdisciplinary team members
- Coordinate care transitions and community resources
- Provide compassionate support and education to patients and families
- Master's or Doctorate in Nursing
- Board Certification as a Nurse Practitioner
- Eligible for Missouri licensure
- Graduate of an accredited Physician Assistant program
- NCCPA certification
- Eligible for Missouri licensure
Candidates should have a strong level of comfort discussing goals of care and serious illness management. Experience in hospice, palliative care, oncology, geriatrics, or related fields is strongly preferred.
