Position Summary

Under the leadership of the department Director and Assistant Director the MDS Coordinator/LVN is responsible for the development and completion of MDS (minimum data sets) accurately and in a timely manner as required for reimbursement by Medicare, state and federal regulations, as well as hospital policy and procedures. The MDS Coordinator will work with an interdisciplinary team to coordinate patient assessments and treatment plans and will complete assessments within the timeframe of patient stay, and submit data to Medicare. The MDS Coordinator possesses skills and knowledge to competently provide appropriate patient information to Medicare.

Experience

One year experience as an LVN is required. Preferences include: Two years of current experience with MDS in a healthcare setting with strong working knowledge of state and federal regulations covering long term care and skilled nursing care. Knowledge of MDS computer software program and MDS certification strongly preferred.

Education

Level of education sufficient to obtain CA LVN license.

MDS Certification preferred

Must pass orientation and initial competency assessment prior to independent assignment of patients requiring the applicable skill, and must complete and pass all annual competencies.

Licensure/Certifications

State of California LVN license required. American Heart Association Healthcare Provider BLS required.
