• Certifications Required: ACLS, BLS, PALS, NRP, CHEMO, TNCC, EFM, Tech Cert Verification - National, Residency Declaration Form, State License 1 Verification, NURSYS Verification (RN, LPN/AP)

Job Details
• To qualify:
• MUST START BETWEEN NOV 11 - DEC 11
• MUST WORK 48 HOURS PER WEEK
• No Holiday RTO - Must be willing to work the Holidays: Thanksgiving, Day after Thanksgiving, Christmas Eve, Christmas Day, New Years Eve, & New Years Day
• 8 Week Assignment
• NEED: RN Med/Surg Tele
• "Pertinent Unit Info: float requirements, weekend requirements, ratio- first to float, every other weekend, no call, 1:5-6 nurse patient ratio.
• EPIC EMR

Job Requirements
• BLS
• Must be able to read & interpret Telemetry independently.

Schedule Information
• SHIFT: Night shift, 3/12's, Every other weekend. Every other holiday.

Unit Specific Information
• NEED: RN Med/Surg Tele
• Pertinent Unit Info: float requirements, weekend requirements, ratio- first to float, every other weekend, no call, 1:5-6 nurse patient ratio.

Additional Details
• APN Candidates: I-9 and E-Verify Attestation Form is required.
• * Hospice, Home Health, or anyone required to drive for their position: Requires a copy of driver's license with them at all times. This does not expire for the client.
• Required: Of note, the "date declared" on the form should be the date that the candidate began living in the state, not the date the form was signed.
