Summary:

Provide public health nursing and related services to individuals, families, and the general community to promote wellness and prevent illness.  Actions are directed toward the goals of prevention, assessment, risk reduction, and health status improvement for individuals, families and communities. Activities include, but are not limited to, systematic analysis of health data, care plan development, health education and advocacy, coalition building, and collaborating with community partners to promote the health of a population. Continuation of this position is contingent upon funding allocations. 


Responsibilities:

The RN- Community/ Public Health position performs the following essential functions within the framework of the Public Health Core Competencies (PHCC). These duties and responsibilities are not an all-inclusive list but rather a general representation of the duties and responsibilities associated with this position. The duties and responsibilities will be subject to change based on organizational needs and/or deemed necessary by the supervisor.

Responsibilities:

* Conducts process and outcome evaluation activities.
* Analyzes public health data and communicates findings to a variety of audiences.
* Participates in the development, implementation, data analysis and report writing of the Community Health Assessment (CHA) and Community Health Improvement Plan. Communication Skills
* Assess current performance of the organization in all areas of the Division to lead quality and compliance efforts and work collaboratively to analyze designated program performance trends and help to facilitate quality improvements through various methods such as: Staff development efforts, procedure development and implementation, quality improvement efforts, compliance/program reviews and provision of resources.
* Utilize data in the promotion of nutrition health and wellness: qualitative, quantitative, primary, and secondary.
* Conducts and reports on program evaluations to improve program effectiveness and demonstrate value to programmatic and funding partners.
* Comply with regulatory and mandatory reporting requirements at the tribal, local, state and federal levels. Facilitate compliance with regulatory and accreditation standards.
* Evaluate policies and procedures for health impact.
* Disseminates information to the public through presentations, news releases, newsletters, web sites, social media, demonstrations, media interviews, and displays.
* Coordinates the activities of meetings or events.
* Ability to operate business computers and office equipment, including in a Windows environment, specifically Word, Excel, Access, SharePoint, Teams, and Presentation software.
* Communicate effectively in the English language, both verbally and in writing.
* Develops print materials that are health literate and utilizes design skills.
* Plans, implements and teaches relevant educational programs.
* Reaches and engages diverse audiences in ways that are inclusive and do not discriminate to ensure full access to programs, facilities and educational services.
* Interact with customers, community partners and co-workers with fairness and equity and deliver services free of bias and prejudice.
* Participates in planning and assessment meetings with both internal and external stakeholders.
* Educates key stakeholders on key public health initiatives and plans.
* Collaborate with partners for promotion of wellness and disease prevention.
* Contributes to team efforts to accomplish results as required.
* Participates in local, regional and statewide public health events, committees and coalitions as needed.
* Maintain working relationships with all departments and external programs partners
* Provides surveillance, investigation, and public information during disease outbreaks.
* Collaborates with other agencies in biological, environmental and weather emergencies. 25.
* Identifies and/or develops program materials for use by consumers and other professionals for various teams within Oneida Community Health Services Department.
* Seek out and use evidenced based and best practices in programming.
* Meet financial deadlines and follow purchasing processes.
* Ability to work with minimal supervision.
* Actively support the department and Division mission, vision, and strategic plan goals and objectives.
* Offer creativity and innovation in addressing and responding to emerging program and customer needs

* Bachelor's Degree in Nursing, Registered (BS/RN)

Must pass a pre-employment drug screen and adhere to the Nation Drug and Alcohol-Free Workplace Policy.

A Tuberculosis (TB) Screening and/or a 2 step TB Skin Test is required within thirty (30) days of employment.

Must provide a copy of diploma/degree, license, or certification upon employment.
