Summary

Job Summary: Oversees and provides guidance on therapeutic diets and service of meals for patients in hospital, clinic, or other healthcare facility by performing the following duties. Conducts a nutrition assessment and obtains nutrition history from patient when pertinent.Consults with physician and other health care personnel to determine patient's nutritional needs and diet restrictions, such as low fat or salt free.Formulates menus for therapeutic diets based on medical and physical condition of patient, and integrates patient's menus with basic institutional menus. Develops and implements plan of care based on assessment of nutritional needs and available sources, and correlates plan with other health care. Runs diet report and prioritizes inpatients according to nutritional needs before beginning daily rounds. Assesses skilled patients and develops nutrition care plan for skilled patients and within established time frame. Schedules and performs outpatient teaching within established time frame. Bills for services and sends report to ordering PCP. Consults with physician/nurses regarding unclear orders and clarifies needs in with dietary staff. Maintains and organizes files and materials for individualized instruction. Documents and performs food/drug interaction, teaching as needed. Attends care plan meetings, dietary meetings, and gives input on nutrition issues at PRMC. Initiates 24-72 hour calorie count if one is needed/ordered and records calories and protein for 3 days. Instructs patients and/or family members as soon as possible and/or within 3 days of receiving a diet referral. Develops and recommends feeding and supplement schedules. Develops instructional materials that are clear, up-to-date, concise, and includes samples of meals. Along with Dietary Manager checks supply of commonly ordered supplements and re-orders as needed through Pharmacy on special items. Calculates diet and meal patterns and educates dietary staff on changes. Documents 90-day reviews for all LTC residents at consulting facilities. Provides consulting services as outlined in contracts. Provides in-services as requested by Sodexo and consulting facilities. Communicates nutritional care plan and other significant information to appropriate persons. Notifies supervisor of all unusual incidents occurring within area of responsibility. Maintains confidentiality and demonstrates ethical conduct as outlined by hospital policy so no valid complaints occur. Instructs patients about meal plan and dietary changes and establishes goals and objectives to help meet the plan. Documents nutrition care plans and information into patient's medical record.

Education/Experience: Fifth year college or university program certificate; or two (2) to four (4) years related experience and/or training; or equivalent combination of education and experience.
