Job Summary

Under the general supervision of the ACT Team Leader/Clinical Supervisor the ACT RN will provide direct community based ACT (Assertive Community Treatment) Services to individuals whom have been diagnosed with serious and persistent mental illness and are homeless or at risk for becoming homeless. This includes the responsibility of providing Medication Education Services in addition to conducting nursing assessments, assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing a range of treatment, rehabilitation, and support services.

Accountabilities

*Provide medication education services to patients on an individual, group, or family basis in the office and in community settings to teach about medications, side effects and medication management as identified in consumer treatment plans.

*Under the direction of the team psychiatrist and in collaboration with other nurses on the team, organize and manage the system of administrating medications to patients and assuring integration of medication education into patients' treatment plans. Under the direction of the team psychiatrist, manage pharmaceuticals and medical supplies. Develop, revise and maintain medication policies and procedures.

*Provide phone support to patients during non-business hours. i.e. evenings, weekends and holidays.  Provide crisis response services including assessment, intervention and stabilization follow up.

*In collaboration with the team psychiatrist, coordinate, schedule, and administer medical assessments of consumers' physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment and medical treatment.

*Complete a comprehensive nursing assessment of all patients' physical health and dental health history.

*Act as liaison and consult with families and community agencies to maintain coordination of the patient's medical treatment process.

*Assist and support patients to have and effectively use a primary care physician, dentist, and other medical specialists as required.

*Provide ongoing assessment, problem solving, side-by-side services, skill training, supervision, prompts, assignments, monitoring, encouragement, and environmental adaptations to assist consumers with activities of medication and medical health.

*Instruct, assist, and support consumers in developing basic living and social skill areas that are essential for independent living including: interpersonal communications; community resource utilization and integration; relapse prevention; budgeting; healthy lifestyle skills and practices; transportation; medication monitoring; symptom management; employment related skills and transitioning to community living.

Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

Associate's Degree in Nursing. BSN preferred.

Two (2) years of psychiatric/mental health nursing experience.

Required Licensure/Registrations/Certifications

Licensed to practice as a Registered Nurse in South Carolina

Must have a valid driver's license for South Carolina.

ANCC certification in Psychiatric Mental Health Nursing-Preferred

Knowledge, Skills and Abilities

*Previous nursing experience with SPMI population. Extreme personal responsibility to ensure availability and readiness to respond to emergencies or crisis situations both during shifts and while on-call.

*Must be able to work various shifts, including an on call rotation of nights / weekends.

*Must feel comfortable in delivering community-based services. 

*Ability to provide direct clinical services to consumers on an individual, group, and family basis in the office and community settings, along with teaching behavioral symptom management techniques to alleviate and manage symptoms.  

*Driving is required for this position using the employee's personally owned vehicle.  Employee must have reliable transportation and will be expected to adhere to the Prisma Health Driver Safety Policy and specific department driving policies.
