This is an excellent opportunity for an advanced practice provider (APP) to join a collaborative and supportive APP/physician practice at the UW Health Cancer Center at ProHealth Care. This is an established community cancer program, fully accredited with Commendation by the American College of Surgeons Commission on Cancer and QOPI® Certified.
UW Health and ProHealth Care are in collaboration to provide breakthrough therapies, promising clinical trials, state-of-the-art technology and exceptional patient care to people with cancer and hematologic malignancies. This ensures that care is comprehensive and easy for patients to navigate.
The APP provides comprehensive, evidence-based care to improve the lives of people with cancer and hematologic malignancies. Care is provided in the ambulatory and hospital settings. The APP is involved in the triage and outpatient care of patients, including acute care visits, management of infusion reactions, chemotherapy and immunotherapy pre-treatment visits, palliative care, symptom management and survivorship care. The APP rotates to the hospital, as a consulting provider. There is collaboration with a strong interdisciplinary team, with the APP providing coordination of care, education and support for each patient and their family.

What You Will Do:
The Lung Cancer Screening Pulmonary Nurse Practitioner (NP) in an advanced practice role in collaboration with Pulmonary Physicians and Thoracic Surgeons. As the lung cancer screening NP, you will manage the daily operations and workflow of this program. You will be instrumental in guiding patients through the screening process, ensuring they receive comprehensive care and support. This includes working with referring physician offices and data mining in EPIC to manage patients' follow up testing. By leveraging your expertise, you will help the patients coordinate their care and contribute to improved outcomes for our patients.
Responsibilities:
The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Major duties and responsibilities of the position:
- Conducts EPIC mining for eligible patients
- Outreach to patients who are eligible for the lung cancer screening program
- Schedule lung cancer screenings and follow-up appointments based on recommendations
- Outreach to patients already in program to ensure follow-up recommendations/requirements are being met
- bility to communicate effectively regarding the lung cancer screening program and its benefits
- Will conduct virtual visits with patients to discuss the lung cancer screening program and follow-up
- Enters orders and interprets labs, tests, and diagnostic studies
- Partnership with radiology/radiologist to discuss recommendations and follow up as needed
- Partnership with oncologists when patient is referred
- Willingness to reach out to referring providers to discuss their patients results, outcomes and recommendations
- Connects patients with smoking cessation resources
- Connects patients with support services as needed
- Demonstrates leadership and works collaboratively with the interdisciplinary team.
Requirements:
- Must possess and maintain an active, current, full and unrestricted APN license to practice as an NP
- Basic Life Support Certification (BLS) through the American Heart Association
- Clinic/health management experience
- Excellent interpersonal communication, problem solving and conflict resolution skills
- Possession of analytical decision-making skills necessary to quickly assess
- Working knowledge of EPIC preferred
- Ability to run reports and mine data proficiently preferred
