Description

Reporting to the Director of Education andProfessional Development, this position is responsible for the oversight and managementof the Diabetes Self-Management Education Program. The Diabetic EducationFacilitator works collaboratively as an active member of the interprofessionalteam to provide therapeutic patient and family centered care. The DiabeticEducation Facilitator will provide education to individuals and families on awide variety of topics with an emphasis on diabetes education. The DiabeticEducation Facilitator systematically and continuously collects and assessesdata related to patient health status to develop, execute and evaluate the planof care. He/she utilizes evidence to drive clinical decisions and practicerelated to individual and population health. He/she facilitates learning,educational experiences, and educational activities for personnel throughdirect and indirect methods. He/she assists the Director of Education with programdevelopment and projects to drive improvements in clinical and professionalrole performance, patient care quality, and cost. The Diabetic EducationFacilitator models professional roles and behaviors that assist personnel inacquiring competency, knowledge, and clinical skills,

EDUCATION/TRAINING/EXPERIENCE:

  • BSN required; MSN preferred
  • Four years of experience in acute care nursing required
  • 3-5 years of experience as a diabetic educator preferred.
  • Demonstrated progressive teaching and communication skills preferred.

CERTIFICATIONS/LICENSES:

  • Current CA State licensure as a Registered Nurse required.
  • Current CPR instructor certification required within 6 months of hire.
  • CBDCE certification required within 6 months of hire

Compensation Information:
$116833.60 / annually - $169416.00 / annually
