Regular Company: Roswell Park Comprehensive Cancer Center Department: Cancer Prevention & Control Time Type: Full time Weekly Hours: 40 FTE: 1 Shift: First Shift (United States of America) Summary: The Patient Care Navigator (PN) provides patient and community support as a clinical navigator to ensure continuous quality of care; assesses, evaluates, and tracks all patients for navigational needs; provides individual assistance to patients, families and care givers throughout the course of screening, diagnosis treatment and/or survivorship; navigates patients through the health care system, including providers' offices, hospitals, insurance and payment systems, patient-support organizations, community resources and other components of the health care system; accesses needed resources and services to allow patients and patient families to move through the spectrum of cancer care into a long-term survivorship; coordinates with internal services and outreach programs to facilitate timely cancer care; works with the resource center to provide necessary patient education and training required by the care team; serves as a resource for hospitals, screening facilities, county health units, state and Indigenous health departments, patients, physicians, and lay public, works with community organizations, NYS Cancer Services, Indigenous Tribal Health Programs, Indigenous Health Services, Federally Qualified Health Centers, and outside health facilities to facilitate potential patients for cancer screening, diagnosis, and or treatment survivorship outreach. The PN also offers support and assistance and provides patient navigator and care management services as needed, tracks patients through health care systems, including primary care offices, hospitals and outpatient clinics, insurance and payment systems, patient-support organizations, cancer services programs and other components of the health care system. The PN will also help identify barriers preventing patients from receiving timely and high quality healthcare treatment and work to find solutions; ensures that continuous quality care is received by all patients; assists with connecting patients to community and clinical referrals for assessment and treatment; assists patients in accessing needed resources and services to move through the spectrum of cancer care; works with primary care providers (PCPs) to coordinate internal and external services, outreach programs, local and regional health centers and transportation services to facilitate care. Serves a s a resource for cancer screening services and management of co-occurring conditions along with PCPs and clinic staff. Uses Electronic Medical Record (EMR) systems to identify and flag patients due for cancer screenings for targeted education and navigation. Work closely with Community Outreach Manager and key stakeholders at clinics to define and execute a workflow meeting the needs of the community and practice. Valid New York State Driver's License.
Education and Experience
1. Bachelor's degree in a health-related field and the equivalent of one (1) year of full-time experience in community, patient or customer oriented support; or
2. Associate's degree in a health-related field and the equivalent of two (2) years of full-time experience in community, patient or customer oriented support; or
3. High School Diploma or High School Equivalency Diploma and the equivalent of four (4) years of full-time experience in community, patient or customer-oriented support.
NOTE:
Required degrees must have been granted by an accredited college or university or one recognized by Roswell Park Comprehensive Cancer Center as following acceptable educational practices.
Preferred Qualifications:
Bachelor's degree in a health-related field and the equivalent of two (2) years of full-time experience in community, patient or customer-oriented support, or oncology support activities; or Associate's degree in a health related field and the equivalent of four (4) years of full-time experience in community, patient or customer-oriented support, or oncology support activities; or High School diploma or High School Equivalency diploma and the equivalent of six (6) years of full-time experience in community, patient, or customer-oriented support or oncology support activities.
