Your Role:

Tetra Tech Inc. is currently seeking a Case Manager for the New Jersey Implementation and Strategy Program. This position will provide technical guidance, strategic direction, and management services during the implementation of case management services.  The Case Manager will be responsible for coordinating and managing project cases, ensuring that client needs are met efficiently and effectively. This role requires strong organizational skills, excellent communication abilities, and a comprehensive understanding of case management principles and practices. The ideal candidate for this role will be in and around the state of New Jersey potentially necessary for travel to a work location on a scheduled basis.   

Project Name: New Jersey Implementation and Strategy Program

§  Case Management:

o   Oversee individual cases within the project, ensuring timely and effective service delivery.

o   Implement strategic case plans, outlining goals, timelines, and resources needed.

o   Monitor and assess client progress, adjusting plans as needed to achieve desired outcomes.

§  Supervisor/Lead Manager Engagement:

o   Serve as a contact for the Case Management Supervisor/Lead, addressing any concerns or issues.

o   Provide regular updates and reports when needed to the Case Management Supervisor/Lead on case management activities and outcomes.

§  Documentation and Reporting:

o   Maintain accurate and up-to-date case files and documentation.

o   Prepare regular reports on case status, progress, and outcomes for the Case Management Supervisor/Lead.

o   Ensure compliance with all relevant regulations and organizational policies and procedures are met.

§  Team Collaboration:

o   Attend daily/weekly team meetings, ensuring the project's needs and expectations are met.

o   Discuss case progress, share best practices, and address any challenges.

o   Work closely with team members to coordinate services and resources.

§  Risk Management:

o   Identify potential risks and issues with cases and develop mitigation strategies.

o   Address and resolve any challenges that arise during the case management process.

§  Continuous Improvement:

o   Identify opportunities for process improvements and implement best practices in case management.

o   Stay current with policies and procedures in the project's case management practices.

·       Conduct activities in line with internal procedures, legislation, and industry standards.

·       Pursue continuous professional development and maintain a high degree of discipline knowledge and awareness.

·       Work in a safe manner at all times and report all health and safety incidents and concerns.

·       Additional duties as required.

Qualifications:

·       Bachelor's degree in Social Work, Psychology, or a related field. Or equivalent experience in Case Management duties.

·       Minimum of 1-3 years of experience in case management.

·       Proven experience in managing complex cases and working with diversity.

·       Strong organizational and time management skills.

·       Excellent communication and interpersonal skills.

·       Proficient in case management software and/or related tools.

·       Strong organizational and problem-solving abilities.

·       Ability to manage multiple cases simultaneously and work under pressure.

·       Ability to work flexible hours, including occasional evenings and weekends, if required.
