Summary of Job Duties: We are seeking a dedicated Nurse Patient Educator to join our team at the Pine Bluff Family Medicine Clinic. This role focuses on promoting screenings, early identification, and clinical navigation for the health and development of children from prenatal through five years of age. The Nurse Patient Educator will be responsible for conducting educational sessions with families to provide information on child health and development. They will also coordinate with healthcare providers to facilitate appropriate screenings and follow-up care. Additionally, the role involves developing educational materials and resources to support families in understanding and managing their children's health needs. Qualifications:
Minimum Qualifications:
- Associate's Degree in Nursing plus seven (7) years of experience as an educator/instructor or RN OR BSN in Nursing plus five (5) years of experience as an educator/instructor or RN with at least one (1) year of experience in a clinical environment.Valid Arkansas Registered Nursing license.
- Current BLS certification OR must successfully obtain certification within the first thirty (30) days of employment.
- Valid AR drivers' license required, with current automobile insurance and reliable car for moderate travel to LR occasionally and to surrounding communities (mileage reimbursed at the current state mileage reimbursement rate) if a state
vehicle is not available. Occasional out-of-state travel. - A personal working cell phone is required.
- Must be able to work evenings and weekends as needed

Additional Information:
Key Responsibilities:
Nurse Case Management & Home Visiting:
- Conduct home visits and clinical assessments as needed to support high-risk clients.
- Identify medical and psychosocial needs and develop client-centered care plans.
- Provide health education and guidance tailored to each family's needs and health literacy.
- Support CHWs in care coordination and referral follow-up to ensure continuity of care.
- Assist families with navigating healthcare systems and accessing appropriate services (e.g., Medicaid, WIC, behavioral health).
Mobile Health Clinic Obligations:
- Prepares patients for clinic visits by obtaining vitals and entering data into the EHR; accompanying patient to assigned exam room; obtaining patient HPI; updating active problems and medication lists, and updating allergies and past surgical and medical history.
- Communicates brief summary of reason for patient's visit to provider.
- Gathers and places supplies and instruments appropriate for anticipated exams and/or procedure, based on stated reason for appointment or information gleaned during the intake.
- Assists the provider with patient care.
- Administers medications and vaccines per protocol or as ordered by provider, performs point of care testing.
- Reinforces patient education and instructions at conclusion of visit.
- Assists patient in scheduling follow-up appointments.
- Assists patients with scheduling appointments with outside providers when outgoing referral is ordered.
- Exhibits competency with professional nursing skills and EHR documentation.
- Exhibits professionalism by maintaining a positive working relationship with patients, providers and employees.
- Assists with cleaning, stocking, preparation, and driving of mobile health units as need
Education & Training:
- Provide ongoing training and mentorship to CHWs on maternal and child health topics, including prenatal care, postpartum recovery, safe sleep, breastfeeding, infant development, and family planning.
- Develop culturally responsive educational materials for families and CHWs.
- Facilitate group education sessions, workshops, or classes as needed.
- Stay up to date on clinical guidelines and public health trends relevant to maternal and infant health.
Quality Improvement & Compliance:
- Participate in data collection, case reviews, and program evaluation efforts.
- Collaborate with the team to identify service gaps and develop strategies to address them.
- Ensure documentation meets HIPAA and programmatic standards.
- Contribute to continuous quality improvement (CQI) projects and audits.
May perform other duties as assigned
