Job Summary:

Reporting to the Nurse Manager the Service Delivery Supervisor works as part of a team to facilitate client care with front line staff and Continuing Care Coordinators. This position will provide supervisory support to the Home Support Services program. Relationship building with funders and Home Support Workers is essential. The Service Delivery Supervisor collaborates with Nursing Managers on issues that require direction. Care is provided within the framework of quality management and the standards and scope of practice set by VON, Continuing Care and the College of Licenses Practical Nurses of Nova Scotia.

Responsibilities:

  • Conduct initial visits for new referrals as per Home Care Standards
  • Performs supervisory visits for CCA's and client specific visits as required
  • Contributes to Continuing Care Assistants (CCA) performance development plan Communication with Continuing Care including but not limited to progress notes and discharge reports and attending case conferences Provide education to CCAs as needed, including orientation Works collaboratively within a multidisciplinary team and acts as a clinical resource and mentor for CCA's

Qualifications:

  • Demonstrated skills in leadership, community nursing, team building, communication and education
  • Understands and uses effective communication methods/tools Computers skills including but not limited to Microsoft Office, Procura,  Excel and Cell Trak Ability to work independently and collaboratively as a member of intra/inter agency teams "LEAN" knowledge an asset
  • Familiarity and understanding of the role of the Provincial Continuing Care Program Registered with College of Licensed Practical Nurses of Nova Scotia
  • Recent experience working in community based health care system
  • Available to work flexible hours
  • A valid drivers license and access to a reliable vehicle
  • Valid CPR C / First Aid required 
  • Must be able to wear Personal Protective Equipment (PPE)
